The Long Term and Post Acute Care (LTPAC) Health Information Technology (HIT) Collaborative was formed to advance HIT issues through coordinated efforts, host an annual Summit and publish a Road Map to advance strategic priorities and inform provider organizations, policy-makers, vendors, payers, and other stakeholders.
